Scope of Services
Think Well provides psychiatric medication evaluation and management by board-certified PMHNPs. We do not provide psychotherapy, crisis intervention, inpatient care, or court-ordered evaluations. Referrals will be made when additional services are needed.
Risks and Benefits of Medication Management
Psychiatric medications can reduce symptoms and improve functioning. All medications carry potential side effects and risks, which your provider will discuss with you before prescribing. No specific outcome can be guaranteed. You have the right to accept or decline any proposed treatment.
Patient Rights and Responsibilities
You have the right to participate in your care, ask questions, access your records, and discontinue treatment at any time. You are responsible for providing accurate health information, attending scheduled appointments, taking medications as prescribed, and treating staff with respect.
Limits of Confidentiality
Your information is protected under HIPAA and Nebraska state law. Exceptions where disclosure may be required include: imminent risk of harm to yourself or others, suspected abuse or neglect of a minor or vulnerable adult, and valid court orders.
Right to Withdraw Consent
You may withdraw this consent and discontinue treatment at any time. Stopping psychiatric medications abruptly may carry clinical risks; please discuss discontinuation with your provider before stopping.
